1) Register Online at: http://registration.hockeycanada.ca/registration/?ID=354
Online Registration is available and is a mandatory requirement to help streamline the Registration process. Please be aware that this Registration is for the Hockey Canada Registry.
Registrants that are living in a rural area need to list their Legal Land Description as their physical address as well as a mailing address when filling out the Online Registration section.
2) Fill out the waivers included in the registration.
Additional forms including the Parent/Player Code of Conduct Form, Zero Tolerance Policy and the Media Release Form are now combined into one Waiver section during the online registration process.
3) Download, fill and email Player Medical Information Form to email@example.com
The player Medical Info Form (PDF version) is also available for download AFTER the registration process is complete and the "Thank You" page is in view. Completed forms can be emailed to firstname.lastname@example.org or dropped off in person during one of the Registration Fee Payment nights.
4) Send Payment: e-transfer to email@example.com password: hockey
Cheque, cash and e-transfers are accepted forms of payment. If e-transfering payment please indicate which player(s) the e-transfer(s) are for and please include the password "hockey" to deposit the e-transfer. Cheques can be mailed to Fort Macleod Minor Hockey Association, P.O. Box 2180, Fort Macleod, AB T0L 0Z0.
Early Bird Registration pricing is only available until Aug 21, 2019 and must be paid and completed in full to qualify.
Respect in Sport
Parents of new registrants(players) must complete the online Respect In Sport-Parent program. If you would like to be a Coach then you must also complete the online Respect In Sport-Activity Leader program which can both be found on the hockeyalberta.ca website.
*Please note that new this year the Respect In Sport programs have an expiration date and must be renewed if not taken within the last 4 years.*
